By revealing and empowering the roles and capabilities of humans lying behind “nothing happened today” or “things went well”, we aim to realize safer systems and society.

In order to maintain a high level of safety of complex systems such as aviation, railways, healthcare, and power supply, it is essential to learn from both failure and success cases. However, despite the saying "One man's fault is another's lesson", it can often be difficult to learn from safety cases, that is, others' experiences. In this symposium, researchers and field engineers participated, discussing human minds and behaviors that hinder or promote learning from case studies.
